Case Manager (2 Positions) The Case Manager will work with the Survivor Care team to provide case management to individuals who have experienced sex trafficking and/or sexual exploitation.
Provide case management (goal setting, employment and education planning, skills-development workshops, and personal support) Fight4Freedom clientele.
Conduct comprehensive needs assessments and develop support plans with clientele to help them build toward their long-term success.
Identify and build partnerships with local housing providers and other service providers
Work with partners in the area of survivor ministry and work with the staff and volunteer team to further develop new and existing ministry areas.
Experience in case management or client support is essential.
Housing Specialist The housing specialist works with the survivor care team to identify housing solutions and to coordinate the development of safe housing plans.
Work with survivors and case managers to identify immediate and long-term housing solutions for survivors of sex trafficking and/or sexual exploitation.
Develop and steward partnerships with a variety of housing providers across the province.
Support research into housing models and opportunities across the province.
Work with the survivor care team to develop and implement a plan to open a home for survivors.
Work with partner housing providers to develop innovative housing solutions.
Work with partners in the area of survivor ministry and work with the staff and volunteer team to further develop new and existing ministry areas.
Previous experience with housing or case management is an asset.
Survivor Care Coordinator
You are responsible for the operations of the survivor care department. That means working closely with volunteers, staff, survivors, service providers, law enforcement, churches, as well as other groups that may want to be involved with Fight4Freedom. You must be flexible with your time as no day/week/month is like another.
You are responsible for working closing with the survivor care staff and volunteer team to (1) help build support plans for case management to determine the best forms of support for survivors through highly individualized services; (2) provide advice and support to the team in navigating relationships with survivors; and (3) work through challenges and troubleshoot with the team. This involves support check-ins with staff and volunteers, as well as training and updating available services and referrals.
You are responsible for meeting with and supporting survivors directly through support and/or case management as needed. This requires a flexible schedule and a willingness to adapt to new situations quickly.
Partnerships are super important in this role. Building relationships with service providers, law enforcement, churches and other groups is vital to creating a good support network for survivors. This involves partnership development meetings and stewarding partnerships well.
You are responsible for maintaining the database of resources/partnerships available for survivor care and sharing this information with the staff team to ensure that we can provide good well-rounded care.
